Discount physicals are being offered through Piedmont Medical Center for NHS Band Members.
- Participant must provide physical VOUCHER from NHS at the time of office check-in.
- Dates and times for discount physical are listed on the voucher.
- Additional voucher can be picked up in the the band office.
- Must pay by CASH ONLY ($20.00)
- Physicals must be on approved South Carolina High School League (SCHSL) forms.
- SCHSL physical forms must be filled out completely and signed by parent/guardian prior to appointment.
- Parent/Guardian must accompany student for physical exam.
